In this study, we showed that midlife dietary fat intake
was related to cognitive performance, especially in
domains of global cognitive function, semantic memory
and psychomotor speed, and to the occurrence of MCI
later in life. To our knowledge, this is the first study to
investigate the association between midlife dietary fat
intake and later cognitive performance. These results are
in accordance with some earlier shorter term follow-up
studies suggesting that dietary saturated fats may have
negative effects, whereas unsaturated fats may have
positive effects on cognition (Kalmijn et al., 1997a;
Morris et al., 2004; Morris et al., 2005).
Our results agree with one earlier study that diet
may have a stronger effect on cognitive function
among women (Lee et al., 2001). The sex differences
in lipid metabolism may partly explain the divergent
results (Mittendorfer, 2005), but also men in this
region tended to die prematurely from cardiovascular
disease (Puska et al., 1983).
In earlier studies higher SFA intake (Laitinen et al.,
2006), and higher energy intake (Luchsinger et al.,
2002) seemed to be associated with a higher risk of
AD especially among the ApoE e4 allele carriers. It
has been previously proposed that the effect of dietary
fats on the lipid and lipoprotein metabolism may differ
by ApoE allele status (Rubin and Berglund, 2002). In
this study, we did not observe any significant ‘e4-SFA’
–interactions.
The mechanisms by which dietary fats may affect
cognitive performance are unknown, but several
hypothetical pathways have been proposed. Vascular
factors might be important mediators as they may lead
to impaired cognitive function through white matter
lesions (Breteler et al., 1994). In our study adjustment
for several cardiovascular risk factors and diseases,
however, did not attenuate the results. Diet high in fat/
SFA may lead to cognitive impairment also through its
effects on for example hyperinsulinemia (Greenwoodand Winocur, 2005), oxidative stress (Zhang et al.,
2005), or endothelial damage (Roberts et al., 2005).
Alterations in the body fat could disregulate estrogen
balance and lead to cognitive changes (Gibbs and
Gabor, 2003). The effect may also be conveyed by
inflammatory mechanisms (James et al., 2000).
Further, in experimental studies SFAs have decreased
the levels of hippocampal brain-derived neurotrophic
factor (BDNF) that are relevant for neuronal plasticity
and the maintenance of cognitive functions (Molteni
et al., 2002). Diet high in SFA and cholesterol may
increase hippocampal accumulation of amyloid betapeptide
(Ab) (Oksman et al., 2006).
